It is Minhog Ashkenaz to remain in schul for Yizkaur.

Is this the minhog of most MMA-affiliated minyonim, or do some practice the Polish custom of leaving for Yizkaur?

In KAYJ we do not say Yizkor, we just have Matnas Yad, and we mention to the congregation that at this time whoever would like to say Yizkor may do so. Therefore no one leaves. On Yom Kippur, although the official Minhag is not to go out, many people go out, since this is the only break (of 2-3 minutes) throughout the day.

Matnas Yad is the Minhag which later caused Yizkor (in Eastern Europe).
Since in the last day of YOT, KRHT finishes "איש כמתנת ידו", the Minhag of Chasidei Ashkenaz was to promise to send money to לומדי תורה ועניי ירושלים עיר הקודש, and to send it after YOT. The nusach appears in the Rodelheim Machzor. Later, the Minhag in Eastern Europe became to mention Neshomos, since one is promising money, so may this be Le'ilui Neshomo.

Just to add to the discussion - In Pfalz and Elzos Yikzor was never said until after 1870. After Germany annexed these portions in that year, some of the customs of southern Germany (Which included yikzor) crept in. It was not universally said in Elzos and Pfalz, but was not uncommon either. From what I understand, since Churban Europa most Alsatian kehillos do say yizkor. Apparently this is due not only to the churban, but to accomodate the influx of Jews from the east after the end of the cold war.
