1) Does one say "Ono" or "Eloukaynu Vayloukay Avousainu" - "tovou l'fonecho ..."?

2) Does one say "V'Al kulom Eloukah s'lichous" or does one say "V'Al kulom s'lach lonu, m'chal lonu, kaper lonu"

3) Does one say "V'Al kulom ..." four times?

4) In what order does one say "V'Al Chato-im"? Does one say "V'Al Kulom" afterward?

1) The nusach all year around is Elokeinu Velokei Avoseinu Tovo Lefonecho etc., and on Yom Kippur it is Ono Tovo Lefonecho etc.
The Divrei Kehillos (pg 235-237) discusses this point.

2,3) According to Nusach FFAM, one says Ve'al Kulom Slach Lonu after Uvelo Yodim and after Simhon Levov (after Azus Metzach nothing is said), and before Ve'al Chato'im Sheonu Chayovim Aleihem Arba Misos Beis Din - the full nusach is said (Ve'al Kulom Eloka Selichos etc.).

4) The order according to Nusach FFAM is printed on the bottom of the page in the Rodelheim Machzor - after Oleh Veyored one says Oshom, then Oshom Tolui, then Kores, then Miso Bidei Shomoyim, then Malkus Arboim, and last - Makas Mardus.
