I understand that according to Minhag Frankfurt, one only says Shoumer Yisroel in Tachanun both at Shacharis and Mincho on a Taanis Tzibbur. The end of the third Shoumer Yisroel is not "HaM'shalshim b'shilush k'dushoh l'kodoush" but "HaM'shalshim kodoush, kodoush, kodoush". Is this correct?

Minhag Frankfurt is to say Shomer Yisroel on Tanis Tzibbur at Shacharis only. But on Tzom Gedalyoh Shomer Yisroel is said also at Mincho.

The nusach in the last Stanza is המשלשים בשלוש קדושות קדוש קדוש קדוש.

In Washington Heights during WWII, Rav Yosef Breuer zt"l instituted saying Shomer Yisroel every day after Tachanun by only the Chazzan while still seated followed by the Y'hi Ratzon tefillos normally said after Krias HaTorah on Mondays & Thursdays. On Mondays & Thursdays the Y'hi ratzon tefillos were said in their normal place.

What you mention of R' Breuer Z"tl is true. Yet, that is not the old Nusach. Some of what R' Breuer added in various places does not conform to the old minhagim - and R' Schwab told R' Hamburger to stick to the old nusach and not include new changes from R' Breuer.

Yes, I am aware that this was not the normal custom. In fact, Rav Breuer only meant it as a Hora'as Sha'a for the duration of the war. When asked after the war about its continuation, he replied that the situation in Eretz Yisroel is no better and it should continue. Rav Breuer, as the Rav of his Kehilla, was vested with the authority to make such takanos for his community. I did not mention it to indicate that we should do the same.

During the selichos, is shomer yisrael said at the end? What about at tachanun of shacharis of selichos days?

Shomer Yisroel is said at the end of the Selichos, but not at Shacharis of those days, except for Tzom Gedalyah.
