Hoshana minhogim on Hoshana Rabba was created by Daniel
Is the Minhag to use three Hoshanos or five? Is the Minhag to beat the Hoshanos three times or five? On the ground or on a Kli (it would seem that five Hoshanos, five beatings, and specifically on the ground, is Minhag Ari and not Ashkenaz)?

There were different ways, all options - 3 Hosahanos, 5 or 7, and different amounts of hitting are valid. One can hit on the floor or other places.

I have been in a MA shul where the הושענות are waved when saying the words קול מבשר מבשר ואומר like one does with the ארבע מינים in הלל. Has anyone come across this Minhag or know of a source?

The R'mo (in Siman תרס"ד Seif ז) says that while holding the Hoshanos one shakes them. Some people may have understood that this should be done like the Lulav.
