Borei Nefoshos was created by DStrauss
What is the proper nusach for Borei Nefashos? The older editions of the Sfas Emes and the Avodas Yisrael say that it is "al kol ma shebara", while the newer edition of the Sfas Emes says "al kol ma shebarasa". Additionally, all of them place "bahem" in parentheses, should this be said or not?

According to Dr. Jonah Frankel (who has been working on a definitive siddur of the Rishounim for some time now), the correct text is as follows:

"... bourei n'foshous rabous v'chesrounon al kol ma sheboroso l'ha-chayous bohem (bohen) nefesh kol choy. Boruch (atoh HaShem) chai ho-oulomim:

Concluding "Bourei Nefoshous" with a brocho concurs with the Gro as well as being in line with other "Birchous Hanehenin".

In the Rishounim of Ashkenaz we find all 4 options, so it's hard to say which is more accurate.
The Or Zorua and Ravyoh say Sheboroso, and the Tashbetz, Hagohous Miemoeni and the Rosh say Sheboro. And we can find both with Bohem and without Bohem in the Rishounim.

Does one conclude Borei Nefoshous with a brocho? "...nefesh kol choy: Boruch Ato HaShem (Melech) Chai/Chay HoOulomim" as is the Nusach HaGro?

Regarding the two גירסאות of שברא vs. שבראת it seems that this is tied to the issue of whether or not to be חותם with ברוך אתה הי . The Gro holds to say שברא just like he holds to finish with ברוך אתה ה because he holds it is a ברכה ארוכה therefore like other "long b'rochos" the מטבע is לשון נוכח, נסתר, ונוכח whereas those who hold to say שבראת hold that it is a ברכה קצרה and as such the מטבע is לשון נוכח.

Minhag Ashkenaz is not to conclude with a Brocho but to conclude Boruch Chei Ha'olomim.
