What is the Minhag concerning taking on one's father's name? R' Hirsch's name was Shimshon, but he was called Shimshon Refael. Is this Minhag widespread? When one gets an aliyah, are they called "ben" the father or the grandfather?

The Minhag of using the father's name was mostly practiced in the 19th century. A person would use his own name for the Aliyoh Latouroh and in Mi Sheberach's etc. (so Rav Hirsch for example was called "Shamshon Ben Rapha'el"), and only when introduced in public etc. he would use his father's name. Therefore, when calling after someone who passed away, only his name is to be used and not his father's name (so when calling after Rav Hirsch you would call the child "Shamshon", or "Shimshon" according to today's more common pronunciation).
