What's the minhog regarding the brocho on the challoh on Shabbos (and other days)? Does one person say Hamoutsi for all? Or after he says a brocho, each one make their own too? Does the ba'al habayis say "birshus" before the brocho?

There are two known minhogim. Either the ba'al habayis says the brocho for everyone, or each individual says the brocho on the piece he gets.
In the Minhogim in Yiddish, it says that the ba'al habayis says Birshus and is Motzi everyone. On the other hand the Chavos Yo'ir says that the minhog is that everyone says the brocho on his own.
Rav Yedidyah Tiah Weil says that the minhog is that everyone is yotzei from the ba'al habayis, and that this was the minhog of his father the Korban Nesanel. We see from that that there was also a different minhog at that time as well.
Today the most common minhog is that the ba'al habayis is motzi everyone in the brocho.
