Proper Place and Procedure for Mishabayrach for Cholim

On Shabbos, where according to nusach ashkenaz is the meshebayrach for a choleh made:

1) Is it done after each aliya if the oleh needs to make it?
2) Is it done after the 6th aliyo and if so why?
3) Done after the 7th aliyo and if so why?
4) Is it done before Hagboh?
5) Is a separate one for men and women made with the Gabbai saying it and inserting the name provided by the individual(s)
6) Is it said together by the Zibbor with each person inserting the name and if so when?
7) Does the gabbi, Chazzan or Rov make it?

I have seen all of the above done.

Mi Sheberach was not said for sick people on Shabbos, since Shabbos Hi milizok, and we do not want to cause sadness on Shabbos. The only case a Mi Sheberach for someone sick was said, is if there was danger he would pass away on that day, on condition that no proper Mi Sheberach had been said for him earlier. There was then a special nusach said for him.
The Mi Sheberach (on weekdays or on Shabbos in the case mentioned above), is done after ones Aliyoh.

What about more choulim than there are Aliyous?

What about for a chouleh who is unable to attend shul maybe due to their illness?

What about women who are choulous and either do or do not have husbands who are present?

It is quite rare to have even one Mi sheberach Lechole on Shabbos (in the criterias mentioned above), and it definitely isn't common to have more Cholim than Aliyos.

It seems that in case there is need of Mi Sheberach Lechole and none of the people who got the Aliyoh did the Mi Sheberach for him, the Mi Sheberach is done after the Mi Sheberach for the last Oleh (Maftir) is done. Anyone who needs Mi Sheberach can get it there, although it doesn't seem that it was done as frequently as today.

For weekday Mishebayrach L'Choulim:

Is there a difference in regard to Youm Touvim, Purim, Chanukoh, Fast Days and Shabbos in regard to the Mishebayrach L'Choulim?

Is it only recited on Monday and Thursday or may it be recited on other weekdays as well?

May the Mishebayrach be said for both men and women together?

There will be a chapter in Shorshei Part 5 about the Mi Shebeirach.

Yom Tov is similar to Shabbos, it seems that in other days you mentioned one could say Mi Sheberach as in weekdays.

Generally, Minhag Ashkenaz held that one does not say many Mi Sheberach's for sick people (as one does not say many Kadeishim and many Brochos), therefore if a person who is sick and in need of Mi Sheberach had a Mi Sheberach said for him once - it was not said again.
Therefore Mi Sheberach Lechole wasn't something very common, unlike some places today that after Krias Hatorah you have a long list of names (many of them mentioned for many weeks already), and therefore there isn't much discussion about men and women together etc.

I understand that this subject will be covered in the upcoming volume of S.M.A.. If one requests a Mishebayrach L'Couleh for someone is having surgery, does one do so before the surgery or afterwards while that person is recovering?

Usually the time to say the Mi Sheberach would be before the operation. After the operation he could be mentioned in a regular Mi Sheberach.
