Bracha on the Torah during aliyah

What is the proper procedure when one is called to the Torah for an aliya according to minhag ashkenaz? For example, where does one kiss with the tzitzis? At the beginning of the aliyah in the sefer Torah and at the end? Does one leave the scroll open or roll it closed when saying the bracha before the aliyah and afterwards? Does one lift the staves/aytzim during a particular word in the bracha? If you could not only explain the procedure, but the reasons as well, I would appreciate it.

Open the Sefer Torah to see the verse that the aliya has to start with and kiss that spot with the zizis held in ones hand. Hold the sefer torah with both hands (say silently etz chaim hee etc.) - torah remains open. Turn your head to the left and make the bracha out loud. When the Ba'al Koreh begins to read remove your left hand from the sefer (etz chaim) and leave the right one still grasping it the entire time the alyoh is being read. The Oleh should read along (silently) with the Ba'al Koreh word for word. After the aliyoh is read, take the zizis and kiss the verse that was just concluded, roll the sefer together and make the brachos.

I saw the inscription of the brakhah after Torah's reading in an old siddur is "אשר נתן לנו תורת אמת, חיי עולם נְטָעָהּ בתוכנו"
That is "hei" with dagesh in נְטָעָהּ is correct?
